• Visits are free if your doctor accepts Medicare assignment, but extra services may incur costs.

Visit Details

• Includes a "Health Risk Assessment" questionnaire to develop a personalized prevention plan.
• Covers routine measurements, medical history review, prescription overview, custom health advice, future health planning, and a preventive services schedule.
• Optional assessment of social factors affecting health is available.
